CHECKLIST NAVIGATOR FOR AIR AMBULANCE
Angel Air International simplifies the process of long distance medical transportation for patients, family members, and Case Managers/Social Workers.  Transportation of this type requires substantial coordination and correspondence between professionals who understand the authorization processes required in transferring medical patients between sending and receiving facilities.  We simplify this process by providing our customers a downloadable Dispatch Information Form to gather necessary information, then all you have to do is simply fax in your request.  The information will allow our Flight Coordinators to begin developing cost effective transport options and quotes for the level of services available for the patient.  Once we evaluate your request, we will quickly make initial contact for any clarifications.   
  
FLIGHT PREPARATIONS
Upon acceptance of the quotation, we will send over an authorization to proceed outlining the level of service, legal and HIPAA notices, any financial agreements and the flight itinerary.  The medical transport team will prepare the selected aircraft and make contact with the sending facility to obtain any patient reports or updates.  The Pilot in Command will perform due diligence weather briefings and obtain any Notices to Airmen (NOTAM) to assess conditions to execute the safest flight plan for the trip.  All "Go-No/Go" flight decisions shall be determined by the Pilot in Command while medical oversight and supervision remains under the control of our Medical Director in accordance with Department of State Health Service standards.
 
International Transfers
Please be prepared to provide for each passenger; legible copies of a current non expired passport, visas and travel documents. Its best if you can provide an electronic copy via e-mail.  If you don't have a copy of the passport; Please be prepared to provide the following; Full Legal Name, Date of Birth, Place of Birth, Passport Number, Passport Expiration Date and Home Address.

Medical Health Insurance Coverage
When the patient has medical health insurance, please be prepared to provide all information requested in the Dispatch Information Form:

Luggage
Passenger is allowed only one carry-on size suit case. Any intended excess baggage must be communicated to the Flight Coordinator for us to accommodate such additional requests.

What you should expect when contacting Angel Air International:

  • You will be greeted by professional Flight Coordinator 24 hours a day, 7 days a week, 365 days a year. 
  • The information we gather will help us determine the most economical mode of transport, the type of aircraft, and composition of the flight team and ground services required. 
  • Once the costs and itinerary are finalized, you will be asked to provide a credit card to guarantee payment. We accept cash or bank wire funds in place of a credit card payment but all payment arrangements are made ahead of the scheduled flight. 
  • Photo identification is required prior to boarding the airplane.  All international flights require a passport and any applicable paperwork prior to boarding. 
  • Your medical transport service includes all transportation logistics for the patient and the accompanying family member. There may be some exceptions involving ground ambulance transportation if a second family member is traveling with the patient.
